Liability Waiver

Safety is Village Yoga’s number one priority. We want to know that you are 100% capable of
participating in the yoga activities (hereinafter activities) offered by village yoga.

Waiver and Release: In consideration for my participation in the activities provided by Village
Yoga. I assume all risks associated with such participation. I recognise that these activities
require physical exertion and I am fully aware of the risks and hazards involved. I understand
that these risks, hazards and dangers may include bodily injury due to physical exertion and the
unavailability of immediate medical attention. I understand that it is my responsibility to consult
with a physician prior to and regarding my participation in the activities provided by Village
Yoga. I represent and warrant that I am physically fit and I have no medical condition that would
prevent my full participation in these activities. I agree that if I am injured, or become ill while
participating in or as a result of having participated in the activities offered by Village Yoga
(and/pr my heirs) will not bring claim against, sue, attach the property of, or prosecute any
person or entity in any way connected with Village Yoga or its partners, employees, agents, or
contractors, even though the injury, or damages, known or unknown, or may be caused by or
result from negligence or carelessness on their part.

In addition, I release and discharge all persons and entities in any way connected to Village
Yoga and all employees, agents, or contractors, from any and all actions, claims, or demands
which I now have or may in the future have for any injury or damage caused by or resulting from my participation in the activities offered by Village Yoga, even though the injury or damage may be caused by negligence or carelessness on their part. I agree that the foregoing waiver and release binds me and my heirs, distributors, guardians, legal representatives, successors, and assigns. I further grant Village Yoga permission to use for promotional purposes photographs, video footage, recording, or other records of my participation in the activities offered by Village Yoga in which my image or likeness appears. I understand that registration fees for classes are nonrefundable.

I have carefully and completely read the foregoing, and I understand what I have read. All of my
questions have been answered to my full satisfaction. I am aware that this is a waiver and
release of liability and that I am giving up certain legal rights. I sign below of my own free will.
